Privacy Policy & Statement of Information Practices
Last updated: June 2026
1. Introduction
Eyeology Management Inc., operating as Eyeology (“Eyeology,” “we,” “us,” or “our”), respects your privacy and is committed to protecting it. This policy describes how we collect, use, disclose, and protect the personal information of patients, customers, and website users (“you”), and the choices you have.
This policy applies to information we collect, use, or disclose: on our website at eyeology.com and any related media, channels, or applications connected to it (collectively, the “Website”); in email, text, and other electronic messages between you and us; and when you interact with our services.
Privacy laws in Canada generally define “personal information” as information about an identifiable individual — information that on its own, or with other information, can identify, contact, or locate a single person. Personal information does not include business contact information used to communicate with you in a professional capacity. “Personal health information” is a special category of personal information about your health and care, and is addressed in Section 10 (PHIPA) below.
Please read this policy carefully to understand how we handle personal information and personal health information.
2. How our Website works
Our Website provides information about our eye-care and eyewear clinics, services, locations, and franchise opportunities. Some functionality is provided through third-party platforms:
- Appointment booking. The Website does not book or process appointments directly. When you choose a clinic and select “Book appointment,” you are directed to that clinic’s online booking page (a Google Calendar appointment-scheduling page) or to contact the clinic directly. Please do not include medical details, symptoms, medications, or health-card information in a booking request. Bookings made there are handled through Google and the clinic’s own systems, not through this Website.
- Forms. General-inquiry and franchise-inquiry forms on the Website are processed by our website host. We ask that you do not submit medical or personal health details through any Website form (see Section 10).
- Clinic locator and maps. Links to clinic locations open in Google Maps so you can find directions and clinic information.
When you use these third-party platforms, the collection and use of your information is also governed by the privacy policies and terms of those third parties. This policy does not apply to information you provide to, or that is collected by, any third party. The Website may include links to third-party websites and services we do not control; we encourage you to read the privacy policy of every website you visit.
3. Information you provide to us
- Identity and contact details you enter in a form — such as your name, email address, phone number, and selected clinic.
- The content of any message or inquiry you choose to send us (for general, eyewear/retail, franchise, partnership, media, or website-support inquiries).
- Records of correspondence if you contact us.
Website forms are intended for general, eyewear/retail, franchise, partnership, media, or website-support inquiries only. They are not for appointment booking, medical information, symptoms, diagnoses, medications, health-card details, or urgent eye concerns.
4. Information we collect automatically (cookies & similar technologies)
As you navigate the Website, we and our service providers may use cookies, web beacons, and similar technologies to collect certain information automatically, including details about your device, browser, IP address, pages viewed, links clicked, and how you interact with the Website. A cookie is a small file placed on your device; you may refuse or delete browser cookies through your browser settings, though some parts of the Website may not function properly as a result.
Analytics, advertising, and tracking tools
We may use third-party tools to understand how the Website is used and to measure our marketing, which may include Google Analytics / Google Tag Manager and the Meta (Facebook) Pixel. These third parties may use cookies and similar technologies to collect information about your online activity over time and across websites. We do not use advertising or remarketing pixels on appointment-booking or inquiry pages. You can control cookies through your browser and opt out of many advertising cookies through the Digital Advertising Alliance of Canada (youradchoices.ca).
5. How we use your information
- To respond to your inquiries and coordinate with your selected Eyeology clinic.
- To direct you to a clinic’s booking page and help you find a location.
- To follow up on franchise, partnership, media, or retail inquiries.
- To operate, maintain, secure, measure, and improve our Website and marketing.
- To comply with legal and regulatory obligations.
- For any other purpose with your consent.
6. How we disclose your information
We do not sell your personal information. We may disclose personal information we collect or that you provide:
- To the Eyeology clinic you select, so it can respond to and coordinate your request.
- To service providers who perform functions on our behalf, including website hosting and form processing, appointment-scheduling and mapping (Google), and analytics/advertising measurement — where we have appropriate contractual, privacy, and security arrangements in place, and only for authorized purposes.
- To legal, accounting, compliance, or regulatory parties where required or appropriate.
- To comply with a legal obligation, enforce our terms, or protect the rights, property, or safety of Eyeology, our patients, or others.
- In connection with a business transaction, such as a merger, financing, or sale of assets.
7. Cross-border processing
Some of our service providers may process or store information outside your province or outside Canada (for example, in the United States). Where that occurs, your information may be subject to the laws of those jurisdictions. We take reasonable steps to require service providers to protect personal information consistent with this policy and applicable law.
8. Data retention & security
We keep personal information only as long as necessary for the purposes described in this policy, or as required by law, after which it is securely deleted or de-identified. We maintain reasonable administrative, technical, and physical safeguards designed to protect personal information against loss, theft, and unauthorized access, use, or disclosure. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
9. Your choices and rights
- Access and correction. Subject to applicable law, you may request access to, or correction of, the personal information we hold about you. Some information may be exempt from access in limited circumstances permitted by law.
- Withdrawing consent. Where you have consented to a collection, use, or disclosure, you may withdraw consent at any time, subject to legal or contractual restrictions and reasonable notice.
- Marketing. You can opt out of marketing messages at any time using the unsubscribe link in our emails.
- Cookies. You can manage cookies through your browser settings.
10. Personal health information (Ontario PHIPA)
We do not collect personal health information through this Website. Clinical records are collected and held by your Eyeology clinic — the health information custodian under Ontario’s Personal Health Information Protection Act, 2004 (PHIPA) — within its own secure systems. Please don’t submit medical details, symptoms, medications, or health-card information through any Website form or booking request.
To access or correct your clinical records, or to raise a privacy concern about them, contact the clinic that provides your care. You may also contact the Information and Privacy Commissioner of Ontario (ipc.on.ca).
11. Marketing communications (CASL)
Where you contact us through a form, we use your information to respond to your inquiry and coordinate with your selected clinic; that is not consent to receive marketing. We will only send promotional or marketing messages where you have separately opted in, and you may withdraw consent at any time using the unsubscribe link in our emails.
12. Children
The Website is intended for a general audience and is not directed at children. We do not knowingly collect personal information from children through the Website. Pediatric eye care is arranged by a parent or guardian directly with a clinic.
13. How to contact us
Privacy Contact: Privacy Officer, Eyeology Management Inc. — info@eyeology.com. Use this contact for questions about this policy, to exercise your rights, or to raise a privacy concern. Requests about clinical health records should be directed to the clinic that provides your care.
Optometrists providing care at Eyeology clinics in Ontario are registered with the College of Optometrists of Ontario. You may verify a registration or learn more through the College of Optometrists of Ontario. You may also contact the Information and Privacy Commissioner of Ontario (ipc.on.ca) about a privacy matter.
14. Changes to this policy
We may update this policy from time to time. The current version is always posted on this page with its “Last updated” date.
